Effective August 1, 2026. This Notice is provided to each client as of their first date of service.
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
BeFitRx PLLC (“BeFitRx,” “we,” “us”) is required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy of your Protected Health Information (“PHI”), provide you with this Notice describing our legal duties and privacy practices, and comply with the terms of this Notice. If a breach of unsecured Protected Health Information occurs, BeFitRx will provide notification consistent with applicable federal and Washington State law.
How We May Use and Disclose Your Health Information
— Treatment: To provide, coordinate, or manage your care, including consultation with your collaborating physician and, when clinically appropriate and with your authorization, communication with other treating healthcare providers.
— Payment: To obtain payment for services rendered and process billing and payment transactions.
— Health Care Operations: For clinical documentation, quality assessment, provider training, practice management, and administrative activities necessary to operate our practice.
— Business Associates: We may disclose PHI to third-party service providers (“Business Associates”) who perform services on our behalf, including electronic health record systems, telehealth platforms, and payment processors. These entities are required by law and contract to safeguard your information.
— As Required by Law: We may use or disclose PHI when required to do so under applicable federal or Washington State law.
— With Your Written Authorization: Any other use or disclosure will be made only with your written authorization. You may revoke such authorization at any time in writing, except to the extent action has already been taken in reliance upon it.
Telehealth-Specific Information
BeFitRx provides services through secure, HIPAA-compliant telehealth technology. We implement reasonable administrative, technical, and physical safeguards designed to protect your Protected Health Information in accordance with applicable law. While no system can guarantee absolute security, we take reasonable and appropriate measures to protect your information. Electronic communications may be subject to rare risks of technical failure or unauthorized access.
Your Rights
You have the right to:
— Inspect and obtain a copy of your health record
— Request an amendment to your health record
— Request an accounting of certain disclosures of PHI
— Request restrictions on certain uses and disclosures of PHI
— Request confidential communications by alternative means or locations
— Receive a paper copy of this Notice upon request
— File a complaint with BeFitRx PLLC or the U.S. Department of Health and Human Services Office for Civil Rights (OCR) if you believe your privacy rights have been violated, without retaliation.
Changes to This Notice
We reserve the right to revise this Notice at any time. Any revised Notice will apply to all PHI we maintain, including information previously collected, unless otherwise required by law. The most current version will be available on our website and upon request.
